Replace a single-user Access-based system and manual procedures with
a multi-user LAN-based application to generate customized surgical
procedure forms with detailed case information and to maintain patient,
physician, and procedural data. |

The Medical Forms Management System is deployed at several
Eye-Surgical centers in the NYC metro area. The system features "master
tables" for maintaining ICD-9, CPT, surgical procedure, and other
information common to all the centers. These master tables are embedded
into each software license, thereby eliminating all redundant file
maintenance processes. Data unique to each facility such as physician
and patient is stored in supplemental tables that can be easily maintained
by the local admin staff.
The system has streamlined the generation of specialized
surgical procedural documents by allowing the technician to select
the patient, physician, and scheduled surgical procedure via drop-down
info boxes. The set of medical forms (as defined by the surgical
procedure) is merged with physician and patient information and
printed.
